Is there a way to do the equivalent to an LCD reset using the CLI?
load factory-default is close but requires that you set the system root-authentication before a commit and I would like to return the EX4200 to the true factory default without having to go the the LCD.

Log in to UNIX shell (start sh) and delete all config files from /config and /config/db/config (not sure about the latest location, on all junos it is /var/db/config. something else on EX. You can locate the fiel with find / "*config*" -print.

No, you have to disable that and probly renumber the vc member number to zero.
